All paperwork is now available for download.Paperwork is also available at each community service office, just to the right of each door in hang-files on the wall. Students will need:

• Bloomfield Hills Schools Community Service Contract - pdf
  The contract indicating where a student is going to do service. It needs the signatures of the student, a parent or guardian, and the agency supervisor.
• Agency Evaluation - pdf
  An evaluation to be filled out by whoever directly supervised the student while he or she did service.
• Student Evaluation - pdf
  An evaluation to be filled out by the student when he or she has finished his or her service.
• Written Evaluation - pdf
  Students are required to write a written reflection evaluation about their experiences in community service.
