The registration process will take place in the main lobby of Lahser High School
on the following dates and times:
August 14 (Thursday) |

REQUIRED FORMS FOR REGISTRATION |
Students and/or parents must fill out completely and bring with them the following information contained
in this packet in order to register:
White and Yellow Emergency Information Form (this duplicate form is only available in the Lahser office )
Required to be filled out, up –to-date and on file in the Main Office. Students will not be able to attend school without this information.
The Communications Release Form
Used to determine what information may be released for school district and media purposes.
The Electronic Information Access and Use Form
Outlines the procedures and expectations for student and staff use of District Computers, the Internet, and related technologies.
Student Contract for School Events
Outlines policies and procedures for school events including Homecoming, dances and other school sponsored events.

Code of Conduct Release Form - This form signed indicates that you have reviewed and read the above Code of Conduct information. This form signed is part of the required registration process.

Athletics Physical Examination, Parent and Student Approval Form
This form must be completely filled out and returned to the athletic director or trainer PRIOR to the student participating in any practices or contests. It is required each school year for any student participating in sports.

Opt Out Form for Military Recruiters
Should be completed and turned in deny access to directory information for military recruiters under Revised School Code, MCL 380.1139
Parking Permit Student Application (Juniors and Seniors Only)
Requires a copy of student’s current copy of student class schedule, a copy of a valid drivers license, copy of vehicle registration card, current proof of insurance, parent signature on application, and a $25.00 fee made out to Bloomfield Hills School District.
PTO Welcome Letter • Membership & School Directory Form
PTO membership entitles each family to two 2008-2009 Lahser school directories.
PTO Volunteer Form
Volunteer for one or two of the many activities listed in this form. PTO needs your help and support to make certain that our High School remains the exceptional school it is.
Senior Open Lunch Permit (Seniors Only)
Outlines Open Lunch policies and procedures and requires signature of both student and parent.
Transportation Request Form 2008-09 - If your high school student needs bus transportation, this form is needed.
