In the near future, all paperwork will be available for download. For the present, all required paperwork is available at each community service office, just to the right of each door in hang-files on the wall. Students will need:

• A Bloomfield Hills Schools Community Service Contract - pdf
  The contract indicating where a student is going to do service. It needs the signatures of the student, a parent or guardian, and the agency supervisor.
• An Agency Evaluation - pdf
  An evaluation to be filled out by whoever directly supervised the student while he or she did service.
• A Student Evaluation - pdf
  An evaluation to be filled out by the student when he or she has finished his or her service.
• Essay Guidelines - pdf
  Students are required to write a reflection paper about their experiences in community service.
